On-line Applications

The International Consultants of Delaware, Inc. (ICD) provides applications for all of its evaluation programs in Adobe PDF format. In order to read and print the files, you will need to download them to your computer and view them in Adobe Acrobat Reader, Version 5.0. If you do not have Adobe Acrobat Reader, or have an older version than Version 5.0, you can download the latest version from the Adobe web site, free of charge. (See link at bottom of page.)

Once you have printed an application packet from the internet, please remember to read all instructions very carefully before filling out the forms, neatly and completely. Completed forms and payment need to be mailed to ICD.

REMINDER: All applicants should check the Required Documents page to find out what country-specific documents they will need to obtain for their evaluation package.

Download the application packet for the ICD Credentials Evaluation (General). Read all instructions very carefully before filling out all forms neatly and completely. Once all forms are completed: 1) Send the completed Application (pages 3-7), all enclosures and your payment to: International Consultants of Delaware, Inc., P.O. Box 8629, Philadelphia, PA 19101-8629 USA.  2) Forward all "Request for Academic Records" forms to the appropriate educational institution(s); 3) Forward all "Request for Validation of Registration/License" forms to the appropriate registration authority(ies).
